Waiver of Liability and Assumption of Risk Agreement

• Please read carefully and complete the following legal document which includes a release of liability

  
  I. PARENTAL CONSENT

  I, the legal parent/guardian of a participant in Experience Church Kids/Youth activities, hereby grant permission for his/her participation in any and all athletic and recreational activities.

  
  II. RELEASE FROM LIABILITY

  I agree to assume all risks and hazards incidental to participation in athletic and recreational activities. I do hereby waive, release, absolve, indemnify, and agree to hold harmless, Experience Church, the staff, ministry directors, volunteers, and participants, for any claim arising out of an injury to my child, whether the result of negligence or any other cause.

   

  III. MEDICAL RELEASE

  Because your child is involved in an activity involving games and recreational activities, there may be an occasion where an injury occurs that requires medical treatment and we are unable to contact you. This situation may occur before, during, or after our scheduled activity while at our site.

  
  I hereby grant permission to Experience Church to administer first aid, secure proper treatment, and/or hospitalize my (son, daughter, ward) in case of emergency, provided they are capable of communicating with me, and according to their best judgment.

  
  I HEREBY ACKNOWLEDGE BY MY SUBMISSION THAT I HAVE READ, UNDERSTOOD, ACCEPTED, AND AGREED TO THIS DOCUMENT.
