RELEASE AND WAIVER OF LIABILITY

I. PARENTAL CONSENT

By checking the consent box on the registration form, I, the undersigned parent/legal guardian of the above-named participant, give permission for my child to participate in the Experience Church youth trip and related activities, including transportation to and from Busch Gardens, athletic activities, recreational activities, and group events.

I understand that participation in these activities may involve certain risks, including but not limited to accidents, illness, transportation-related incidents, slips and falls, weather-related conditions, and injuries resulting from recreational activities.

II. ASSUMPTION OF RISK AND RELEASE OF LIABILITY

I knowingly and voluntarily assume all risks associated with my child’s participation in this event and related activities.

On behalf of myself, my child, and our heirs, assigns, and representatives, I release, waive, discharge, and agree to hold harmless Experience Church, its pastors, staff members, ministry leaders, volunteers, agents, and representatives from any and all claims, demands, damages, causes of action, or liability arising out of or related to participation in this event, including transportation to and from the event, except in cases of gross negligence or intentional misconduct.

III. MEDICAL AUTHORIZATION

In the event of an emergency where I cannot be reached, I authorize Experience Church staff, volunteers, or designated leaders to obtain medical treatment for my child, including first aid, emergency medical care, hospitalization, or transportation by emergency personnel if deemed necessary.

I understand that reasonable efforts will be made to contact me as soon as possible.

I accept responsibility for any medical expenses incurred on behalf of my child.

IV. ACKNOWLEDGMENT

By checking the consent box on the registration form and signing below, I acknowledge that I have carefully read this Release and Waiver of Liability, fully understand its contents, and voluntarily agree to its terms.
